Referring a Patient

Professionals wishing to make a referral to Ashley can contact our Admissions Office 24 hours a day, 7 days a week at 1-800-799-4673 extension 213 and speak with an Admissions Counselor.  The Admission Counselors are trained to answer your questions about the programs we offer and admission criteria. 
In order to expedite the admissions process, it is useful to have the following information about your potential patient ready to share with the admission staff:
  • Demographic Information (name, age, address, employment, gender)
  • Substance use history
  • Brief medical and psychiatric history including names of current medications
  • Name of a family contact person
  • Name of the insurance company and phone number, subscriber   information and employer (so benefits for substance abuse can be obtained)
  • Click here to complete the Referring Professionals form.
Note: Medical records including history and physical, medication administration record, progress notes and labs will be requested for referrals that are currently in another inpatient setting or have significant medical or psychiatric issues.
 
At some point during the pre-admission process, we will request to speak directly with the potential patient.  At that time we can gather additional information and answer any questions they may have about entering treatment. 
 
Once the necessary information is collected and it is determined that we are the appropriate facility, an appointment for admission will be set. The admissions process is typically one to ten days.  Thorough patient information streamlines the process.
 
It is our policy that referring professionals are contacted upon admission and then again by the assigned counselor.  Patients are asked to sign information consent forms upon admission.
 
Insurance information:
Father Martin’s Ashley is considered an in-network provider for most Blue Cross/Blue Shield, Value Options, ComPsych, Managed Health Network and United Behavioral Health PPO plans. Plan benefits are verified prior to admission. Authorization for an inpatient admission is typically required and is based upon medical necessity criteria established by your plan. Please contact the Ashley Admissions Department for more detailed information regarding insurance benefits, medical necessity criteria and the authorization and admission processes. 410 273-2213 or admissions@fmashley.com.
